Affinity Home Care is seeking a Private Duty Caregiver on a contingent basis in Tri-country Area. How you’ll make an impact as a Private Duty Caregiver

• Work one-on-one with patients when and where they need the most help – in their home!
• Empower patients to live safely and productively in their home and complete the tasks they want to do and need to do every day
• Help patients and families adapt to their environment to improve participation in daily activities and improve quality of life

Responsibilities

• Assist clients with activities of daily living (ADLs): bathing, grooming, dressing, toileting and mobility.
• Provide companionship and emotional support.
• Prepare meals and assist with feeding. (if needed)
• Perform light housekeeping: laundry, dishes, vacuuming and trash removal.
• Offer medication reminders. (no administration unless licensed)
• Monitor and report changed in client’s condition to the supervisor or RN.
• Maintain accurate daily notes and documentation of services provided.
• Follow the care plan established by the care coordinator or supervising nurse.
• Adhere to infection control and safety guidelines.
• Respect client’s rights, property and privacy in accordance with HIPPA.

Qualifications

• High school diploma or equivalent preferred.
• Minimum 2 years of previous experience (private home or facility)
• Ability to pass background check.
