- Provide services in speech pathology or audiology in accordance with the physician’s plan of care
- Assist the physician in assessing the client’s level of functioning and the development (and revise as necessary_ of the plan of care by applying appropriate test for speech, hearing and language disorders
- Record, and report the clients' reaction to treatment and any changes in the clients' condition
- Instruct and advise the client, family and other health team personnel in the phases of speech pathology in which they may assist the client
- Checks hearing aids and recommends hearing tests by audiologist when necessary
- Assess the client’s communication, oral-motor integrity, cognitive status, memory, problem solving activities, hearing, speech fluency, and quality
- evaluate and regularly re-evaluate the speech pathology needs of the clientele initiate, develop, implement and make necessary revisions to the client’s plan of care
- Initiate diagnostic, preventative, and rehabilitative procedures as appropriate to the client’s care and safety. Make referrals to other disciplines as indicated by the needs of the client or document rationale for not doing so
- Teach, supervise and counsel the client and family regarding home procedures and other care needs as appropriate to the client’s condition.
- Master’s degree in speech pathology with a current license
- License to practice in the state of Michigan
- Minimum of two years of speech pathology experience

- Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
- Provide reminders and assistance with medication administration in accordance with the patient's care plan and under the nurse's guidance.
- Monitor and record vital signs including temperature, pulse, blood pressure, and respiration to ensure the patient's health and safety.
- Observe, record, and report the clients' reaction to treatment and any changes in the clients' condition
- Effectively communicate with the care team, including registered nurses, licensed practical nurses, and supervisors, to report any concerns, updates, or changes in the patient's status.
- Valid Certified Nursing Assistant (CNA) certification.
- Previous experience in home care, long-term care, or a similar setting is preferred.
- Excellent interpersonal and communication skills.
- Compassionate and patient-oriented approach.
- Ability to prioritize tasks, work independently, and adapt to changing client needs.
- Basic computer skills for documentation purposes.
- Must be physically capable of performing the duties required, including lifting, bending, and standing for extended periods.

- Implement skilled physical therapy services in accordance with physician's plan of treatment
- Assist the physician in the evaluation and assessment of the client's level of functioning by applying appropriate tests and assist in the development and revision of the plan of care
- Treat the client to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical therapy plan of care.
- Observe, record, and report the clients' reaction to treatment and any changes in the clients' condition
- Assess and regularly re-evaluate the needs of the client to develop and revise the client's plan of care
- Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living.
- A current license as a Physical Therapist in Michigan
- Bachelor's or Doctorate Degree in Physical Therapy required
- Minimum of two years' experience as a physical therapist

- Implement occupational therapy services in accordance with physician's plan of treatment
- Assist the physician in the evaluation and assessment of the client's level of functioning by applying appropriate tests and assist in the development and revision of the plan of care
- Treat the client through use of therapeutic activities designed to restore function and self care activities for improving function
- Observe, record, and report the clients' reaction to treatment and any changes in the clients' condition
- Assess and regularly re-evaluate the needs of the client to develop and revise the client's plan of care
- Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living.
- NBCOT certified upon hire, but renewal is optional going forward
- Graduate of an accredited program in occupational therapy (BSOT or MSOT)
- Currently registered/licensed in Michigan. Must maintain an active license in good standing throughout employment.
- Minimum of two years' experience as an occupational therapist
